Our services allow seniors to age in place remaining as independent as possible with assistance from their very own caregiver.

We are looking for a full time experienced Care Coordinator to join our very busy team M-F 9: am-5:00 pm and share the On-Call phone. The Care Coordinator will focus on meeting Client needs and requirements by maintaining Client relationships and assuring adequate scheduling with caregivers.

It is important that the candidate applying for this position has the aptitude to engage in marketing, social media and meeting with potential referral sources for Griswold Home Care in our community.

Duties:

•Conduct telephone intakes with new or potential clients

•Analyze the specific needs and requirements of each client.

Refer appropriate caregivers to clients, based upon the specific needs and requirements of each client and the qualifications and experience of the caregiver

•Coordinate all scheduling activities

•Maintain client/caregiver files

•Perform follow up home visits

•Additional duties as needed
