Job Title: File Clerk
Employment Type: Contract
Work Hours: 40 hrs./week
Work site: 100% On Site
Location: 220 Richmond Ave, Batavia, NY 14020
Job Duration: 2 Months

Minimum Education:
High School Diploma or equivalent
Work Experience:
None required; Relevant experience in health information management or medical records is a plus.
Job Duties:
Review diagnostic and therapeutic medical records to ensure completeness and accuracy.
Organize, manage, and analyze medical records to ensure quality, security, and accessibility.
Ensure compliance with accreditation and legal requirements related to medical records.
Monitor facility compliance with confidentiality policies, including HIPAA regulations.
Assist facility staff in resolving medical records maintenance issues and establishing proper documentation terminology.
Provide technical assistance, training, and support to facility and clinical staff on health information management requirements, including coding.
Assist clinical staff in understanding and adhering to health records policies and procedures.
Participate in data collection and presentation for clinical and administrative reviews.
Verify the presence of required components in resident medical records, such as progress notes and laboratory reports.
Identify missing information and communicate discrepancies to the relevant departments.
Coordinate processes for Medicaid funding and ensure accurate clinical documentation.
Communicate with residents, families, and other entities regarding health record information while maintaining confidentiality.
Maintain databases and track resident health information disclosures.
Ensure timely completion of requests for health information.
Investigate and report privacy issues and breaches of Protected Health Information.
Implement legal and regulatory policies and procedures for access and disclosure of Protected Health Information.
Ensure completion of corrective actions in response to internal and external audit findings.
Preferred Skills:
Familiarity with medical records management and health information systems.
Knowledge of HIPAA regulations and confidentiality policies.
Strong attention to detail and accuracy.
Effective communication skills, both written and verbal.
Ability to work independently and collaboratively as part of a team.
Proficiency in database management and data tracking.
Misc. Information:
Shift Days/Time: Monday - Friday, 8am-5pm
Free parking available at the worksite location
No public transportation available in the area
Interviews will preferably be conducted in person but can be arranged via Teams if necessary.
On-the-job training will be provided, with a learning-as-you-go approach
