MAIN FUNCTION\:



The Hospice RN Case Manager provides skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional hospice standards of practice. The Hospice RN Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, and promotes and maintains quality of life for clients/caregivers through direct care, teaching, and counseling in accordance with the interdisciplinary team care plan.



This position is part of the Massachusetts Nurses Association Bargaining Unit at Cooley Dickinson VNA & Hospice.

Qualification:

MINIMUM REQUIREMENTS\:



* Graduate of an approved, accredited school of professional nursing required; BSN preferred

* Current licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts required



* CPR training required.



* Minimum of one (1) year medical/surgical nursing experience required; hospice/home care experience preferred

* Demonstrated excellent verbal and written communication skills required

* Demonstrated strong interpersonal, organizational, and time management skills required

* Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same including competency in pain and symptom management required

* Current drivers license, a good driving record, and reliable transportation required.
